Increase access to best practice, comprehensive outpatient diabetes care for underserved in San Diego county using a chronic care model emphasizing patient self empowerment.

Prevention and Education Committee
Prevent additional cases of Type 2 Diabetes in San Diego County and improving the awareness of prevention measures among the at-risk populations.

Create effective advocacy and policy strategies that support the
SDDC mission.
